Forms
Forms lets you collect information from clients — intake details, consent, feedback — and read the responses in one place. Go to Forms in the sidebar.
Forms is rolling out gradually. If Forms isn't in your sidebar, it isn't enabled for your account yet — let us know if you'd like it turned on.
Create a form
- Click + New form.
- Choose a starting point:
- Start from a template — pick one of the ready-made templates (each shows how many fields it includes). The fastest way to a working form.
- Start blank — begin with an empty form and a name of your choosing.
- Give it a Name (required for a blank form; a template fills in its own name if you leave it empty).
- Click Create.
You'll land on the new form's submissions page, ready for responses to come in.
Right now, the quickest way to a ready-to-use form is to start from a template — templates come with their fields already set up. A visual builder for adding and editing your own fields is on the way.
Read submissions
Open any form to see its Submissions. Each response is listed with when it arrived, whether it's attached to a Linked client or came in as Anonymous, and how it came in (Shared link or Direct). Click one to open the full response in a side panel. If a form has no responses yet, you'll see "No submissions yet."
The status badge beside a form's name shows where it stands — Draft while it's being set up, Active once it's live and accepting responses, and Archived if you've archived it.
Sensitive answers stay protected
Front Desk Central automatically scans your forms for fields that look like they collect sensitive information — Social Security numbers, card numbers, dates of birth, health details, and the like. When it spots one, a banner appears above your forms list:
The field "…" on "…" looks like it collects … Mark it as sensitive to encrypt these answers?
Click Mark sensitive & encrypt to encrypt future answers to that field (past answers stay as they are), or Dismiss if it's a false alarm. It's advisory — nothing is blocked or changed until you choose.
When you read a response, any field marked sensitive shows a 🔒 next to its label with the note "Sensitive — encrypted at rest," so you can tell at a glance which answers are protected.
Archive a form
To retire a form, open its ··· actions menu on the forms list and choose Archive, then confirm. Archived forms stop accepting responses, but you can still open them to read the submissions they already collected.
Where responses go
Responses appear only on each form's Submissions page (above). In-app forms don't send notification emails, and they don't show up under Messages — that inbox and the Notifications setting cover a separate channel: enquiries that come in from your public marketing site, not your in-app forms.
